Drug Test Collection Site Help

Need help locating collection sites in your area? Simply click on the appropriate link based on which lab(s) you have been provided. Although not required, you will have the option to schedule an appointment as well. If you are unable to locate a collection site within 40 miles of your location, please contact us and we can assist you in finding an alternative collection site.

After entering your zip code select "What testing do you need?" Choose: Employer Drug and Alcohol. Next select "Please select all the drug and alcohol tests that apply to your visit.". Then select "Urine" (if testing for a federally regulated position choose "Urine - Federally mandated"). A list of available collections sites will display. Follow onscreen prompts to view directions or schedule an appointment (not required)

After entering your zip code choose "Employment Drug Testing" and hit "Go". A list of collection locations will be displayed. Follow the on-screen prompts to view directions or schedule an appointment (not required).

Request a Copy of Your Report

Pursuant to the Fair Credit Reporting Act (FCRA), at any time you may request a copy of your consumer/background check report which was prepared by Applicant360 for employment purposes. If you were denied employment within the past 60 days based on information contained in your report, you may request a copy of the report free of charge. After 60 days, or if you were not denied employment based on information contained in the report, you may request a copy of the report for a fee (current applicable fee is listed on the Disclosure Form instructions).

To request a copy of your consumer/background check report prepared by Applicant360 for employment purposes, please click on Electronic Disclosure Request Form below to provide the required information and submit your request electronically. If you are unable to access or complete the electronic form, or if you would prefer to submit your request by fax or regular mail, you may download a hard copy of the Disclosure Request form. Dispute

Dispute the Accuracy of Information in Your Report

Note: If you have not already received a copy of your consumer/background check report, your first step should be to obtain a copy (see Request a Copy of Report above) and carefully review it. 

After reviewing your consumer/background check report prepared by Applicant360 for employment purposes, if you believe that the information contained in your report was correct but you disagree with a hiring decision made by the employer/contractor, please contact the employer/contractor directly to discuss their hiring criteria. Applicant360 is not involved in any hiring decisions made by an employer/contractor.

After reviewing your consumer/background check report prepared by Applicant360 for employment purposes, if you believe that the report contains inaccuracies based on the identification information you provided (either to the employer/contractor or directly to Applicant360), you have a right to dispute the information. All dispute inquiries are treated with understanding, consideration, and respect, and are conducted at no cost to you. Dispute inquiries are resolved as quickly as possible, however please allow up to 30 days (the time allowed by law) for completion.
