USCG - DOT Drug Test Order Form

After completing the form below within 24 hours you will receive via email your registered LabCorp WebCOC forms. Simply bring the registered LabCorp WebCOC Form and Photo ID to any of the LabCorp Patient Service Center listed on the WebCOC Form. Drug Tests forms are active for 30 days. After 30 days a $15 fee is incurred to extend the allowed testing period.

PLEASE NOTE: Taking a drug test does not automatically enroll in the consortium. After you receive the drug test result by email you MUST return to the USCG Link and choose "Enroll a New Crewmember".
